The paper reports the case of a 13-year-old adolescent presenting with a right-sided submandibular lymph node swelling in absence of any other systemic symptoms. The histological exam documented Rosai-Dorfman disease (RD). The MRI of the head reported an infiltrative lesion of the nasal choanae that was confirmed by PET/CT and radically excised. In about 10% of cases RD may involve nasal choanae and can be asymptomatic or present with obstructions, epistasis, deformity in the nasal dorsum and hyposmia. By considering a recent international Consensus, the pa-per discusses the diagnostic and therapeutic approach to the localised forms of the Rosai-Dorfman disease.
