The paper describes the case of a 15-year-old teenager with a 3-week history of abdominal distension and pain. She reported regular menstrual cycles and a recent weight gain. On physical examination, her abdomen appeared voluminous, without palpable masses. A pregnancy test resulted negative. An ultrasound scan revealed a giant, multilocular cyst occupying almost the entire abdomen, later confirmed by a CT scan, through which an ovarian cystadenoma was suspected. A laparotomy was performed and a massive amount of mucinous fluid was aspirated from the cyst. Then, an annessiectomy was carried out with the complete removal of the mass. Overall, the entire mass weighted 12 kg. Histopathology confirmed the suspected diagnosis and cytology tested negative for malignancy.
