Acute liver failure is a rare and complex disease that may present with a worsening course and needs a rational diagnostic and therapeutic intervention, in order to avoid most dangerous complications such as encephalopathy and metabolic changes, which may quickly lead to exitus. Often the only treatment is liver transplantation that can be performed if negative prognosis signs are recognised with appropriate timing and if the patient is promptly sent to an appropriate centre. The paper presents two cases that had similar diagnosis, but different treatment and prognosis.
