The future of paediatrics in Italy needs answers to some questions. How is it possible to regain efficiency considering that several predictions expect the number of paediatricians to begin to decline significantly within the next five years (from a peak of 15,000 down to 11,000)? In what way can the overlap of interventions between the primary and secondary care services be solved? How will it be possible to give back to the primary care service the ability of dealing with that 80% of problems that, according to the WHO, primary care is supposed to solve? Does primary paediatrics need to acquire additional competences or other types of collaborations in order to be able to face it? The present article suggests a guide through these issues.
