Rare, atypical and a great mimicker: the osteoid osteoma you do not expect

An 11-year-old girl presented with limping and right hip pain, initially mistaken for a rheumatological disease (i.e. arthritis) due to the presence of joint fluid. Further investigations showed the real cause: an osteoid osteoma.

When the purpura appears after surgery

The case shows that Henoch-Schönlein purpura may present with isolated abdominal symptoms and mimic a surgical acute abdomen and may sometimes lead to exploratory surgery before the typical skin manifestations appear.

Green vomit and family dynamics: a case of Munchausen by Proxy

Key words: Recurrent vomiting, Bilious vomiting, Munchausen syndrome

The paper reports the case of a 6-year-old girl obsessively brought for medical evaluation by her father due to vague symptoms and recurrent vomiting (at times biliary, documented only through photographs provided by her mother). Extensive medical investigations consistently yielded negative results. The mother levelled serious allegations against her husband (poisoning and abuse), which were later proven to be unfounded. The final diagnosis is Munchausen Syndrome by Proxy.

When pain leads to self-harm

A 16-year-old boy with Cornelia de Lange syndrome and severe sensory disabilities presented with a sudden worsening of self-injurious behaviour. After ruling out common gastrointestinal complications associated with his condition, a thorough dental examination revealed severe cavities and gingival lesions.
