Un test immunoenzimatico rapido e semplice per ricercare gli anticorpi anti-transglutaminasi umana nella malattia celiaca

A simple and quick immuno-enzymatic test to search for human anti-transglutaminase antibodies in celiac disease

Aim of the Work 
The antigen that is recognised by anti-endomysium IgA antibodies (EMA) has been recently identified in human tissue transglutaminase (hr-tTG). Many immunologic tests have been developed in order to search for anti-transglutaminase antibodies: they include both radioimmunologic tests and ELISA techniques. However, they require a specialised laboratory and at least 2 weeks before the family paediatrician can obtain their results.
We developed and assessed a quick immuno-enzymatic test (DIA) to search for anti-hr-tTG antibodies - both IgA and IgG - in serum and whole blood.
Methods 
Our group cloned, expressed and purified hr-tTG starting from intestinal biopsies of celiac subjects. DIA is based upon the absorption of hr-tTG as a spot (0.1 g/spot) on Immobilion-P sheets, which are then treated to block the antigen on their surfaces. These sheets are incubated for 5 minutes with human anti IgA and IgG antibodies conjugated with alkaline phosphatase. After a short washing up, they are incubated with a solution which contains the substrate, then dried and examined; the results are given by colorimetric reaction.
Twenty celiac patients in a florid phase (2 of them with IgA deficit) were assessed with DIA and with ELISA in order to search for anti hr-tTG antibodies (both IgA and IgG).
Results  
All florid celiac patients, including the 2 patients with IgA deficit, were positive both for ELISA and for DIA. The group of 45 healthy controls showed a false positive with DIA and no positive with ELISA.
Conclusions
In terms of effectiveness, the DIA test and the ELISA method are equivalent. DIA is simple and could be used as a first non-invasive screening on a blood drop by family paediatricians.

Introduzione e scopo del lavoro
La malattia celiaca è usualmente sottodiagnosticata. E' legata alla presenza degli alleli HLA DQA1*0501/DQB1*0201. La malattia è legata alla produzione di un auto antigene, indotta nei soggetti predisposti alla assunzione di glutine. Questo autoanticorpo, un tempo identificato come anticorpo antiendomisio (EMA o AEA) è in realtà la transglutaminasi tissutale tTG.
L'utilizzo degli anticorpi antitransglutaminasi di cavia (GP-tTG) è insufficientemente specifico.
Il nostro gruppo ha messo a punto un test ELISA più specifico contro la transglutaminasi umana (h-tTG) ricombinanate hr-tTG, amplificato a partire dalla biopsia intestinale di un paziente con celiachia, usando un primer specifico e clonando il cDNA con un vettore di espressione nei batteri.
Il test è risultato più sensibile e più specifico rispetto a quello con GP-tTG e a quello con AEA, fornendo una spiegazione per i rari casi di celiachia AEA negativi.
Esso però richiede, come gli altri test ELISA, un laboratorio specializzato e dei tempi di diagnostica non brevissimi. Abbiamo sviluppato e valutato un test immunoenzimatico rapido (DIA)b per ricercare gli anticorpi anti-hr-tTG sia IgA che IgG nel siero e nel sangue intero. In quest'ultima forma esso potrebbe essere usato come primo screening non invasivo nell'ambulatorio del pediatra di famiglia

Materiali e metodi
Il DIA è basato sull'assorbimento dell'hr-tTg, in forma di spot (0.1g/spot) su dei fogli di Immobilion-P che poi vengono trattati per bloccare l'antigene sulla loro superficie.
I fogli vengono incubati per 5 minuti a temperatura ambiente con i campioni di sangue o siero del paziente e per 5 minuti con anticorpi anti-IgA e anti IgG umane coniugati con fosfatasi alcalina. Dopo un breve lavaggio i fogli vengono incubati con la soluzione contenete il substrato, vengono asciugati ed esaminati per valutarne i risultati, espressi da una reazione colorimetrica.
Venti pazienti celiaci in fase florida (tra cui 2 soggetti con deficit di DGA) e 45 soggetti sani) sono stati esaminati sia con il test DIA, sia con la metodica ELISA, ricercando gli anticorpi anti-hr-tTG, sia IgA che IgG.

Risultati
Tutti i pazienti celiaci floridi, compresi i 2 con deficit IgA, sono stati riconosciuticome positivi sia dalla metodica ELISA che dal test DIA. Nel gruppo dei 45 controlli sani si è avuto un falso positivo colò test DIA e nessuno col test ELISA

Conclusioni
Il test DIA ha un'efficacia equivalente al test ELISA. E' una metodica semplice e potrebbe essere usato come primo screening non invasivo, su goccia di sangue, ad opera dei pediatri di famiglia
