The authors investigated by echo-doppler 86 patients, aged 4-30 years, with systolic murmur and 31 control patients. In all patients with systolic murmur the presence of either tricuspid or mitral valve insufficiency was shown. Conversely, most but not all patients with valvular insufficiency present a systolic murmur and no murmur is audible in patients without ventricular-atrial reflux. These findings show that systolic murmurs may be clinically benign but they do correspond to a precise hemodynamic phenomenon.
